What Does the Membership Plan Include?

For $349 per year, pediatric patients receive:

• 2 prophylaxis/periodontal maintenance cleanings
• 1 set of x-rays
• 2 preventive check-up exams
• 1 fluoride treatment
• 15% discount on all other dental services (excludes dental implant and Invisalign procedures)

• Once the annual fee is paid, you will be entitled to the dental benefit membership program for 12 consecutive months.

• Patients have the right to cancel at any time by submitting a written request; however, this plan is non-refundable.
